Submit Your Story and Join Club CF

The focus of CLUB CF is LIVING, BREATHING and SUCCEEDING. CLUB CF wants those who have CF or are affected by the disease to see that despite all the challenges that come along with cystic fibrosis, it is possible to live a happy and successful life.

Club CF shows how people in different age groups (20+, 30+, 40+, 50+, 60+, Caregivers) are succeeding. People with CF are making a difference in the world in high school, college, sports, careers, relationships, starting a family, post-transplant and disability. Through Club CF, they can give hope and inspiration to those who are hesitant or nervous about what lies ahead of them.

If you are one of the many people with CF who is LIVING, BREATHING and SUCCEEDING, join Club CF and show the world what you have done!

How to apply:

To submit your story for Club CF, please download an application (PDF l DOC). Complete the application and mail it with any supporting materials to: Boomer Esiason Foundation, Attn: Club CF, 483 10th Avenue, Suite 300, New York, NY 10018.

You also may email a paragraph or two about yourself to ClubCF@esiason.org. Please include your age as well as information about your activities, community involvement, goals and how you stay compliant.

We'll review each submission and contact you to get any additional information we need to put together your profile.
